Licensed Practical Nurses (LPNs) - Multiple Locations

At CHP Home Care & Hospice, our mission is to provide comprehensive home health and hospice services to the people of Northwest/West Central Ohio. CHP is currently seeking part-time Licensed Practical Nurses (LPNs) for home health care, working primarily with infants, toddlers and young people on a case-by-case basis.

As a community-based, nonprofit organization each CHP office is deeply rooted in its local communities and is an integral part of not only the health care delivery system, but part of the local fabric. We are proud to serve Northwest and West Central Ohio for more than 40 years, and look forward to bringing comfort and healing to friends and neighbors for years to come.

SALARY:  Competitive Hourly Wage

 HOURS:  Part-time

 LOCATION:  Positions available in: Ada, Celina, Delphos, and Wapakoneta

Examples of duties include: 

· Assists in carrying out the plan of care as directed by the physician and/or the Registered Nurse, as well as performing the independent functions expected of an LPN

· Is instrumental in educating patients to be self-sufficient in their personal health care

· Reports any change in the patient’s condition to the appropriate Registered Nurse

· Maintains accurate and legible clinical records

· Participates in in-service programs

· Refers to other services as needed

MINIMUM APPLICANT REQUIREMENTS

· Must be a graduate of an accredited Licensed Practical School of Nursing

· Must currently be licensed in the State of Ohio

· capable to lift and transfer patients

· Maintain CPR certification and License

· Excellent communication skills

· Good transportation and proof of current driver’s license

SPECIAL REQUIREMENTS

· One year acute care experience preferred

· Must maintain license by obtaining necessary contact hours and competency
